We’ll Guide You Through Enrollment
Here’s what you need to complete enrollment:
- Immunization Records or Exempt Form
- Print out the forms and have them filled out before your consultation
- Pay the application fee- $100 (non-refundable)
- Fill out the online Pre-Enrollment Form
